Voucher Program

DS International had registered with Arizona Telecommunications Equipment Distribution Program ( AzTEDP ) to provide a large selection of devices at no charge to Deaf, Hard of Hearing and Speech Impaired residents of Arizona .

Our retail store and demonstration center in Glendale , Arizona add convenience to customers for devices pick-up, tutoring and customer service. Please browse AzTEDP website at www.acdhh.org/home/aztedp to get the application form, and order can be placed after they issued you a voucher.

To recieve your equipment, you can come to our store with your voucher for immediate pickup. You can also mail the voucher with signature on the application signature block to:

Once we receive your voucher, you will receive the shipment through FedEx the next day.
